ONLINE RELEASING OF REPORTS OF GRADES & ADVISING SLIP

ATTENTION
Students who are cleared of their accountabilities and have secured final permit for the First Semester, A.Y. 2011-2012 not later than Nov. 02, 2011 05:00 PM from the Accounting Office have the privilege or option to view and print their Report of Grades and Advising Slip These Report of Grades and Advising Slip have specific control and security numbers assigned to each student which maybe considered valid and maybe used during the registration period for the Second Semester, Academic Year 2011-2012.

These Report of Grades and Advising Slip have specific control and security numbers assigned to each student which maybe considered valid and maybe used during the registration period for the Second Semester, Academic Year 2011-2012.

REMINDER: Falsification, alteration or tampering of these forms is considered major and criminal offenses hence they are punishable by expulsion or dismissal.

Note: Please use your C.O.R (Certificate of Registration) as your reference. Please use a 8.5" x 13" (long) bond paper for printing of your Report of Grades For those students with ( Ñ ) in their Middle Name / Barangay, please use ( N ) instead. Example 1: Middle Name: DELA PEÑA => Middle Name: DELA PENA Example 2: Barangay : DASMARIÑAS => Barangay : DASMARINAS Warning: You can only print your R.O.G. and Advising Slip FIVE times.
