Makati Action Center (MAC) Residency Verification Form

MAC Mag-Aaral ng Bayan Certification

National Career Assessment Examination (NCAE) Result

Birth Certificate

Pencil No. 2 (for filling-up of registration form)

NON- MAKATI residents must have a high school average of 88% to quality for admission test

TRANSFEREES

General Weighted average of 88% with no failing grades

Transcript of Records/Certificates of Grades

Honorable dismissal/transfer credentials

• All applicants for Nursing Courses/Imaging Science are being processed by the College of Nursing. Please proceed to third floor, Building 2

Note:

- Non-Makati residents & Transferees are required to have at least 88% of General Weighted Average and must have no failing grades.

- Those who graduated a year or so prior to this school year must present a Certification that his/her Form 137 had not been sent to any other college/university.
