University of Makati - PSA Birth Certificate of Student Applicant
- Voter’s registration ( Registration stub may be accepted ONLY for first-time voters)
- 2pcs Passport picture, white background with name
- Permit to Study (Permit to study from the employer if the applicant is currently working)
